Position Overview

In this role, you will perform comprehensive assessments to ensure suitable placement and care for local populations. Your responsibilities will include delivering critical evaluations and recommendations aimed at improving patient well-being and quality of life. These assessments, primarily conducted in various healthcare settings, are essential for determining the necessary level and scope of care for each individual. Your expertise will be vital in facilitating better health outcomes, providing essential support tailored to the unique needs of each patient.

This position offers a hybrid work model, allowing you to work from home while also traveling to meet patients in your local area.

Key Responsibilities

  • Conduct thorough screenings and evaluations to ensure accurate determinations.
  • Formulate informed care recommendations based on detailed clinical assessments.
  • Perform evaluations in diverse environments, including healthcare facilities and community settings, to address the varied needs of those we serve.
  • Adhere to best practices in mental health, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care.
  • Ensure compliance with regulatory standards while developing care recommendations.
  • Utilize independent judgment and clinical expertise to navigate complex situations, ensuring optimal outcomes for clients.
  • Collaborate effectively with a range of healthcare professionals, patients, and families while monitoring compliance with turnaround time requirements.
  • Leverage technology to manage and track assessment activities efficiently.
  • Stay updated on current literature and regulations related to long-term care and mental health.
  • Maintain quality and efficiency by adhering to necessary workflows.
  • Prioritize and execute tasks with a keen attention to detail.

Qualifications and Skills

  • Active, unrestricted licensure in a relevant healthcare field.
  • A minimum of three years' experience in assessing individuals with mental health disorders.
  • Experience working with individuals with intellectual disabilities or related conditions.
  • Graduation from an accredited institution with a relevant degree.
  • Knowledge of Preadmission Screening and Resident Review (PASRR) requirements.
  • Proficient in assessing psychological and physical functioning to develop appropriate care recommendations.
  • Understanding of psychiatric disorders as outlined by the latest diagnostic manuals.
  • Familiarity with public mental health and medical programs.
  • Demonstrated experience with the geriatric population.
  • Strong communication skills for effective interaction with individuals and groups.
  • Proficient in using computer software and Microsoft Office Suite.
  • Detail-oriented with the ability to manage tasks efficiently within set timelines.
