Responsibilities- Provide direct patient care to residents in a Long Term Care setting, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the healthcare team to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.
- Document patient information accurately and thoroughly in accordance with facility policies and procedures.
- Current LPN licensure in the state of practice.
- 2 years of experience in a Long Term Care or Transitional Care setting.
- Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with the healthcare team.
- Reliable transportation and flexibility to work a variety of shifts, including evenings and weekends.
