Job Title: Social Worker

CrescentCare is seeking a highly skilled and compassionate Social Worker to join our team. As a Social Worker, you will play a vital role in providing assessment, referral, education, service planning, and care coordination services to our clients with a focus on increasing self-management and adherence to behavioral health treatment plans.

Key Responsibilities:
  • Provide assessment and referral services to clients with a focus on increasing self-management and adherence to behavioral health treatment plans.
  • Develop and maintain strong working relationships with case managers, healthcare providers, psychiatrists, and health educators within the agency and the community.
  • Participate in huddles and care team meetings as requested and able.
  • Responsible for ensuring clarity on program expectations and standards, including record keeping and documentation, behavioral expectations, work quality and productivity expectations, effective teamwork, and client-centered ethical practice.
  • Attend all meetings and trainings as directed by supervisor.
  • Attend a minimum of 20 hours of continuing education annually.
Requirements:
  • Master's degree in Social Work (MSW) or related field.
  • Minimum one year of experience providing social work services.
  • Current licensure as a Certified Social Worker (CSW).
  • Reliable transportation and valid driver's license.
