Key Responsibilities
1. Provide palliative nursing care and ongoing support in accordance with the treatment plan and nursing care plan.
2. Collaborate as a team member, demonstrating responsibility to the patient, caregiver, physician, and interdisciplinary team.
3. Implement pain management and symptom relief strategies to ensure patient comfort. Reassess the patient and inform the registered nurse (RN) if pain management is inadequate.
4. Address the physical, mental, social, and emotional needs of terminally ill patients and their families.
5. Comply with all agency policies regarding payroll, billing, and documentation.
6. Exhibit knowledge of medications and their appropriate administration based on the patient's age and clinical condition.
7. Respect and support cultural practices, provided they do not harm others.
8. Ensure accurate documentation in line with company policies and procedures.
9. Refer to interdisciplinary team members and other providers, demonstrating collaboration and responsibility.
10. Assist in educating patients and families about care provisions and services.
11. Participate in Performance Improvement Programs.
12. Engage in in-service programs, team conferences, and staff meetings.
13. Deliver skilled nursing care to assigned patients/families as outlined in the care plan.
14. Contribute to the development of the patient/family interdisciplinary care plan.
15. Respond to the diverse needs of terminally ill patients and their families with respect to their age, religious, and cultural practices.
16. Provide ongoing nursing services, including regular visits as per the patient's care plan and needs, under the direction of the RN Case Manager.
17. Monitor educational needs of each patient and provide necessary education, training, and support with appropriate documentation.
18. Perform additional duties as assigned by the Director of Health Services or Administrator.

- Comprehensive understanding of Hospice Regulations (federal, state, and local).
- Exceptional communication skills, both verbal and written.
- Strong organizational abilities.
- Capacity to manage confidential information.
- Flexibility in work schedule.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
- Current, active, and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice.
