VillageCareMAX is seeking a dedicated professional for a Full-Time position focused on assisting care managers and members in the coordination of services and requests. The Care Coordinator will be responsible for authorizing services in accordance with the care plan and ensuring that these services are effectively initiated and delivered. Key responsibilities include:

  • Addressing inquiries from members and providers concerning health insurance, benefits, eligibility, and authorization processes.
  • Establishing communication with the care management team, provider relations department, and external service providers to fulfill member needs.
  • Documenting, tracking, and following up with the care management team regarding all member service requests.
  • Conducting monthly member outreach and maintaining accurate documentation in the electronic health record system.
  • Supporting the maintenance of documentation, logs, and data entry to oversee activities and outcomes related to Medicare billable services.
  • Reporting any member dissatisfaction and grievances as necessary.
  • Engaging in special projects and performing additional duties as assigned.

We encourage individuals with 3+ years of relevant experience to apply. A high school diploma is required. Strong communication skills, organizational abilities, and problem-solving aptitude are essential for success in this role. Bilingual skills are highly preferred.
