At Spring Gate Rehabilitation and Healthcare Center, we are seeking a highly skilled Wound Care LPN to join our team. As a key member of our healthcare team, you will be responsible for providing total nursing care to residents and supervising clinical staff members in relation to wound care.
The ideal candidate will have a strong background in nursing, with a focus on wound care and management. You will be responsible for assessing, planning, implementing, and evaluating the ostomy care for patients on units, as well as providing advanced clinical guidance to physicians and nursing staff for complex wound drainage and skin protection.

Responsibilities:
- Assesses, plans, implements, and evaluates the ostomy care for patients on units.
- Provides advanced clinical guidance to physicians and nursing staff for complex wound drainage and skin protection.
- Develops and conducts educational programs for staff.
- Determines appropriate nursing interventions for identified patient problems, symptoms, and teaching needs.
- Develops a patient care plan and determines the equipment, materials, and resources needed to successfully implement the care plan.
Requirements:
- Graduate of accredited LPN School of Nursing.
- Valid LPN license in the state employed.
- Two years of nursing experience preferred.
