About the Role:
As a leading provider of mental health services, you will play a crucial role in delivering exceptional care to our geriatric patients facing mental health challenges within the long-term care community. Your expertise will contribute to our mission of improving psychiatric access and patient well-being.
Key Responsibilities:
- Conduct comprehensive psychiatric evaluations to assess residents' mental health conditions.
- Develop and implement individualized treatment plans, including medication management and therapeutic interventions.
- Collaborate with interdisciplinary teams to provide high-quality psychiatric care.
- Prescribe and monitor psychiatric medications, adjusting regimens as necessary to optimize patient outcomes.
- Deliver evidence-based psychotherapy and counseling to residents and their families.
- Maintain accurate medical records in compliance with regulatory standards.
- Participate in quality improvement initiatives to enhance mental health services.
Qualifications:
- Valid Registered Nurse license and completion of an Advanced Practice Nurse Program for Psychiatric Nurse Practitioner.
- Strong knowledge of psychiatric assessment, diagnosis, and treatment.
- Excellent communication and teamwork skills.
- Experience in long-term care settings is preferred.
- Ability to travel within a designated territory.
