Prevents unwanted hospitalizations by meeting patient and family needs in the home. Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. Compliance with applicable laws and regulations with respect to Collaborative Agreements. Completes accurate billing and coding activities for all patient encounters according to CMS guidelines and initiates reimbursement for medically necessary visits in accordance with applicable law and regulation. Provides training and continuing education for staff. Assists in development of clinical practice guidelines/standards in support of quality palliative care. Adheres to the practice of confidentiality regarding patients, families, staff and the organization. Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG. Performs other duties/projects as assigned. What is Required? Masters Degree in Nursing and certification through a national Nurse Practitioner Licensing board. 3 years clinical nursing experience 1 year post-graduate clinical nursing experience in the hospice and palliative care field. Prior experience in Family Medicine, Geriatrics and psychiatry preferred. Current licensure in the state of practice as a Nurse Practitioner and maintain such licensure in accordance with applicable laws and regulations. Board Certification in Palliative Care Preferred. Active DEA license. Able and willing to travel up to 50% as needed to support business and patient needs.
