Job Title: Travel Nurse RN Case Manager

We're seeking a skilled Travel Nurse RN Case Manager to join our team at Medical Solutions. As a Travel Nurse RN Case Manager, you'll be responsible for managing patient care and ensuring the highest level of quality and safety in our healthcare facilities.

Job Summary:

This is a 13-week travel nursing job in Farmington, New Mexico, with 36 hours of work per week. The shift is 12 hours, days, and the employment type is travel. As a Travel Nurse RN Case Manager, you'll have the opportunity to work with a variety of patients and healthcare professionals, and to make a real difference in the lives of those you serve.

Responsibilities:
  • Manage patient care and develop individualized plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals to ensure seamless care and communication.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans.
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure clear understanding and coordination of care.
Requirements:
  • Current RN licensure in the state of New Mexico.
  • Minimum of 2 years of experience as a RN Case Manager.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility and adaptability in a fast-paced environment.
