About the Role

We are seeking a skilled RN Transition Navigator Case Management to join our team at Ochsner Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients through the healthcare system.

Key Responsibilities
  • Manage the care of assigned patients through the healthcare system, utilizing evidence-based criteria and level of care assessments.
  • Develop and implement individualized care plans, working collaboratively with the multidisciplinary team to ensure quality, cost-effective outcomes.
  • Facilitate communication between patients, caregivers, healthcare providers, and third-party payers to ensure a smooth transition of care.
  • Assist with discharge planning, ensuring that patients receive the necessary support and resources to achieve optimal health outcomes.
Requirements
  • Current RN license in the state of practice.
  • 2 years of related experience in case management or a related field.
  • Basic Life Support (BLS) certification through the American Heart Association.
  • Preferred: Certification in Case Management (CCM) or Health Care Management (CPHM).
