Responsibilities- Conduct thorough assessments of patients to identify their physical, psychological, and cognitive needs.
- Develop and implement individualized care plans that prioritize patient outcomes and are based on nursing diagnoses.
- Deliver care in a knowledgeable, skillful, and consistent manner, prioritizing patient needs and available resources.
- Evaluate the effectiveness of care provided and patient progress toward outcomes.
- Identify patients' and their significant others' learning needs and implement appropriate measures to meet them.
- Coordinate interdisciplinary functions to ensure seamless patient care.
- Fulfill all compliance responsibilities related to the position.
- Perform other duties as assigned.
- State of CT RN license
- Zero to six months of experience
- Nursing certification and BSN preferred
This is a full-time position with a 36-hour workweek. The work schedule is evening/night shifts, with weekends as contracted. The department is the Nursing Float Pool.
