Job Summary

Sage Counseling Inc is seeking a highly motivated and experienced Case Manager to join our team. As a Case Manager, you will play a critical role in providing support services to our clients, including maintaining contact through video and/or phone, providing community resources, and supporting their treatment and growth.

Key Responsibilities
  • Provide case management services to clients, including assessment, planning, and coordination of services
  • Maintain regular contact with clients through video and/or phone to ensure their needs are being met
  • Provide community resources and referrals to clients as needed
  • Support clients in their treatment and growth, including setting and achieving goals
  • Collaborate with other professionals, including therapists and counselors, to ensure comprehensive care
  • Document client progress and maintain accurate records
Requirements
  • Bachelor's degree in a behavioral health-related field
  • Minimum 2 years of direct work experience in a helping profession
  • Reliable transportation
  • Bilingual (Spanish) preferred
