We are seeking a skilled Continuing Care Coordinator RN to join our team at HealthEcareers - Client. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions across the care continuum.
Key Responsibilities:- Develop and Implement Care Plans: Collaborate with physicians and healthcare providers to develop and implement comprehensive care plans that address the unique needs of our patients.
- Coordinate Care Services: Work closely with internal and external stakeholders to coordinate care services, including utilization management, transfer coordination, and discharge planning.
- Provide Patient Education: Develop and deliver patient education plans focused on self-management and disease state education.
- Monitor and Evaluate Care: Continuously monitor and evaluate the quality of care provided to patients, identifying opportunities for improvement and implementing changes as needed.
- Collaborate with Healthcare Teams: Work collaboratively with multidisciplinary teams to ensure effective communication and coordination of care.
- Identify Opportunities for Cost Savings: Analyze data and trends to identify opportunities for cost savings and improving the quality of care across the continuum.
- Develop and Maintain Policies and Procedures: Develop and maintain policies and procedures related to case management and care coordination.
- Education: Bachelor's degree in nursing or healthcare-related field preferred.
- Experience: Minimum one (1) year clinical experience as an RN in an acute care setting, plus two (2) years clinical experience as an RN in a licensed home health or hospice agency required.
- Licensure: Registered Nurse License (California) and Basic Life Support certification required.
