Job Summary

As a key member of our team, you will be responsible for overseeing patient care, conducting intake assessments, and providing individual and group services to ensure a cohesive and supportive experience for each patient.

Key Responsibilities
  • Provide individual and group counseling, crisis intervention, psychoeducation, and skills training to patients.
  • Ensure smooth orientation for new clients upon admission, including comprehensive intake assessments to gather history and presenting information.
  • Collaboratively develop and regularly update individualized treatment plans to meet the unique needs of each patient.
  • Utilize patient-centered therapeutic interventions, including motivational interviewing, to enhance treatment outcomes.
  • Document treatment interventions, contacts, treatment plans, and sessions meticulously to ensure accurate and comprehensive records.
  • Maintain an active caseload to keep patients engaged in the program and ensure seamless community reintegration.
Requirements
  • Bachelor's or Master's degree in Social Work, Mental Health, Psychology, or a related field.
  • At least 2 years of experience in a behavioral healthcare setting, serving culturally diverse populations.
  • Preferred qualifications include LCSW, LMFT, LPC, LMHC, LADC.
