Job Summary:

Adoration Home Health and Hospice is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality, patient-centered care in the comfort of their own homes. As a Private Duty LPN, you will work closely with patients and their families to develop personalized care plans that meet their unique needs and goals.

Responsibilities:

Provide specialized care, including trach and G-tube management, to patients in their homes
Collaborate with the care team to monitor patient progress and ensure safety and comfort
Maintain detailed patient documentation and clear communication with patients and families
Administer medications as instructed and document patient files accordingly

Requirements:

Active LPN license in Tennessee
One year of nursing experience, preferably in home health or hospice care
Valid Driver's License and CPR Certification
