Senior Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at CenterWell Senior Primary Care in Mid-City, LA. As a key member of our care team, you will play a vital role in delivering high-quality, patient-centered care to our seniors.
Key Responsibilities- Evaluate and treat patients in accordance with standards of care, utilizing your expertise in primary care and value-based medicine.
- Collaborate with our multidisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, and durable medical equipment.
- Participate in daily huddles and contribute to the development of a positive, patient-focused care environment.
- Document patient care and outcomes in a timely and accurate manner, working closely with our quality-based coder to optimize coding specificity.
- Stay up-to-date with policy and protocol changes, ensuring compliance with clinical leadership guidelines.
- Minimum of two to five years of directly applicable experience as a Nurse Practitioner.
- Successful completion of a Nurse Practitioner program with Board Certification.
- Active, unrestricted Nurse Practitioner license in the state of practice location.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team-based care environment.
- Basic computer skills, including email and EMR.
- Appropriate certification to write prescriptions under the authority of a collaborating physician/medical director.
- Active and unrestricted DEA license.
- Medicare Provider Number/Medicaid Provider Number.
- Experience managing Medicare Advantage panels of patients, with a strong understanding of best practices in coordinated care environments.
- Knowledge of Medicare guidelines and coverage.
