Job Title: Patient Outreach Specialist

At Southeast Primary Care Partners, we are seeking a highly skilled Patient Outreach Specialist to join our team. As a key member of our healthcare team, you will play a critical role in performing Annual Wellness Visits with our Medicare population.

Job Summary:

The Patient Outreach Specialist will work closely with physicians and medical staff to identify gaps in care, safety concerns, and social needs. You will prepare patient charts, communicate patient needs to providers, and conduct outreach to eligible patients who have not yet completed their AWV.

Responsibilities:
  • Maintain patient confidentiality in accordance with HIPAA regulations
  • Attend regular training sessions to stay up-to-date on best practices for conducting Annual Wellness Visits and identifying patient needs
  • Collaborate with the healthcare team to develop personalized care plans for patients based on the findings from their Annual Wellness Visit
  • Utilize electronic health record (EHR) systems to document patient information accurately and efficiently
  • Demonstrate empathy and active listening skills when communicating with patients to build trust and rapport
  • Assist with patient education on preventive care, healthy lifestyle choices, and disease management
  • Participate in quality improvement initiatives to enhance the patient experience and optimize clinical outcomes
  • Maintain a professional and positive attitude while working in a fast-paced, team-oriented environment
Requirements:
  • High School Diploma
  • Healthcare experience preferred but not required
  • Strong organization skills
  • Outstanding communication and interpersonal abilities
  • Friendly and patient personality
  • Comfortable speaking on the phone
  • Comfortable with computer programs and typing
  • Thrives in a high-paced environment
Physical & Mental Requirements:
  • Ability to sit for extended periods of time
  • Ability to stand for extended periods of time
  • Ability to use fine motor skills to operate office equipment and/or machinery
  • Ability to receive and comprehend instructions verbally and/or in writing
  • Ability to use logical reasoning for simple and complex problem solving
  • Ability to travel to multiple locations to support business needs as required
