Responsibilities:- Assess and manage patient health needs, providing comprehensive care and treatment plans.
- Collaborate with physicians and other healthcare professionals to ensure seamless patient care.
- Document patient interactions and medical histories, maintaining accurate and up-to-date records.
- Perform physical examinations, diagnostic procedures, and prescribe treatment as needed.
- Develop and implement care plans, working closely with patients to achieve their health goals.
- Completion of an accredited Nurse Practitioner or Physician Assistant program.
- Current licensure as an advanced practice nurse in the state of Georgia and certification as a Nurse Practitioner or Physician Assistant.
- Minimum of 2 years of experience in Primary Care, with a focus on patient-centered care and collaboration.
- American Heart Association BLS and Healthcare Provider certifications required.
- Competitive salary range: $102,000 to $128,000 per year, depending on experience.
- Eligibility for annual APP incentive plan.
- Student Loan repayment benefit.
