About the Job

The Virtual Registered Nurse (RN) is a key member of the Nursing Care Team, responsible for collaboration, communication, and facilitation of optimal patient care through a computerized and/or digital platform.

Responsibilities
  • Assess, plan, implement, coordinate, monitor, and evaluate patient care from admission to discharge.
  • Utilize clinical knowledge, critical thinking skills, and the principles of case management and adult education to coordinate, implement, and support the plan of care.
  • Ensure seamless, effective, and efficient transition of care across the continuum.
  • Monitor quality metrics specific to the department and ensure complete and accurate documentation in the patient record.
  • Perform roles as mentor, clinician, educator, consultant/collaborator, and navigator.
Requirements
  • Graduate from an accredited professional school of nursing with valid RN licensure in the State of Ohio.
  • Minimum of eight (8) years of experience in health condition management, including adult condition management, with Electronic Medical Record (EMR) experience.
  • Baccalaureate Degree in the Science of Nursing (BSN) degree from an accredited school of nursing preferred.
  • Specialty credentialing and educational degree according to clinical nursing practice specialty area preferred.
