Job Summary:

Trinity Health is seeking an experienced APRN - Nurse Practitioner - Palliative Care to join our team. As a member of our Acute Palliative Care team, you will provide primary or acute healthcare services for specialty health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization.

Key Responsibilities:

Provide high-quality patient care in a fast-paced inpatient setting

Collaborate with interdisciplinary teams to develop and implement patient care plans

Conduct thorough assessments and develop effective treatment plans

Communicate effectively with patients, families, and healthcare teams

Requirements:

Master's degree in Nursing and graduate of an accredited Nurse Practitioner program with specialization in area of practice

State licensure as an Advanced Practice Nurse (APN), Nurse Practitioner (NP) and/or other designation as required by law

Minimum of two years recent and relevant nursing experience preferred

Effective communication and leadership skills
