Overview:
Constellation Health Service is in search of a dedicated RN Clinical Liaison for the Merrimack Valley area. This role is pivotal in facilitating smooth transitions of care for patients. The Registered Nurse Clinical Liaison is tasked with generating referrals for hospice services by fostering strong relationships with social workers, case managers, wellness directors, physicians, and other key stakeholders within the designated territory.
Key Responsibilities:
- Enhancing the quality of care for terminally ill patients and establishing a hospice presence in various healthcare settings including hospitals, nursing homes, and physician offices.
- Developing and nurturing new partnerships with referral sources within the assigned territory.
- Identifying and prioritizing potential referral partners, focusing on decision-makers within each organization.
- Participating in essential meetings at post-acute care facilities and engaging in community rounds as necessary.
- Conducting thorough assessments to determine eligibility for hospice services based on established criteria.
- Providing comprehensive clinical evaluations when required.
- Educating patients, families, and healthcare staff about hospice philosophy, objectives, and available services.
- Ensuring adherence to the hospice care plan.
- Recognizing opportunities for program enhancement and contributing to the development of new initiatives.
- Maintaining up-to-date knowledge of hospice services, admission criteria, and industry changes affecting service delivery.
- Willingness to travel across various geographic areas for assessments, training, and relationship-building.
- Proficiency in electronic medical records (EMR), Microsoft Word, and Excel.
- Performing additional duties as assigned.
- Possession of the necessary licensure, education, and training as mandated by state regulations.
- Exceptional interpersonal skills to manage sensitive and confidential information.
- Demonstrated professionalism, tact, and diplomacy in all interactions.
