We are seeking a compassionate and skilled Registered Nurse to join our Hospice Home team. As a Hospice Home Registered Nurse, you will provide patient-centered end-of-life care in a supportive and comforting environment.
Responsibilities- Implement and coordinate patient care plans with interdisciplinary teams, including social work, Certified Nursing Assistant, volunteers, chaplain, physician, and medical director.
- Focus on symptom management and comfort for patients and their families.
- Utilize excellent interpersonal and communication skills to build strong relationships with patients, families, and healthcare teams.
- Stay up-to-date with current CPR/BLS certification and maintain a strong analytical and problem-solving skillset.
- 1 year of nursing experience in a clinical setting.
- Current CPR/BLS certification through the American Heart Association.
- Excellent interpersonal and communication skills.
- Solid computer skills, including proficiency with Microsoft software.
- BSN degree.
- Home health or hospice experience.
- National certification for Hospice and Palliative care.
- Working knowledge of Epic and state, federal, and JCAHO regulations.
