We are seeking a dedicated and compassionate Hospice Nurse to join our team in Peoria. As a leader in high-quality home health, hospice, and palliative care, we have grown from a single location to over 130 locations across 18 states.

The Hospice On-Call Nurse will respond to calls from patients, families, caregivers, and referral sources, assessing and utilizing critical thinking to provide advice or deliver care. The nurse will effectively interact with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

Key Responsibilities:

  • Assess and coordinate patient/family care to support patients with advanced illness and families in home, skilled nursing facility, or residential care facility.
  • Communicate related to on-call interventions, documenting and communicating to the Case Manager with assessment, plan of care changes, and ongoing needs.
  • Utilize teaching, assessment, and intervention skills to provide comfort care and maximize the quality of life for patients and families.
  • Assess hospice needs, being aware of physical, emotional, and spiritual aspects, and gather data on social, economic, and cultural factors influencing health, well-being, and quality of life.
  • Respond to calls from answering service and voice mail system in a timely manner, triaging patient-related calls by determining needs and responding appropriately.
  • Implement changes to the plan of care as needed with interdisciplinary collaboration, documenting and communicating all calls, interventions, care plan changes, and physician contacts in a manner consistent with policy and procedure.

Requirements:

  • Graduate of an accredited school of nursing.
  • Valid license as a Registered Nurse and current Basic Life Support certification.
  • Valid state Driver's License and reliable transportation.
  • Minimum one year of recent professional nursing experience, preferably in hospice or home health.
  • Good interpersonal skills, computer skills, and ability to work independently.

Environmental and Working Conditions:

  • Works in customer's offices in various conditions, with ability to travel to customer's locations, some overnight stays, and some exposure to unpleasant weather.
  • Staff may be exposed to virus, disease, and infection from patients and specimens in the working environment.

Physical and Mental Effort:

  • Prolonged sitting and some standing are required, with occasional need to lift, pull, carry, and push items weighing up to 50 lbs.
  • Requires working under some stressful conditions to meet deadlines and agency needs, with excellent problem-solving skills.

Essential Functions:

  • Available by phone or pager to meet the needs of patients, as assigned, during assigned shift.
  • Identify and coordinate patient/family care to support patients with advanced illness and families in home, skilled nursing facility, or residential care facility.
  • Communicate related to on-call interventions, documenting and communicating to the Case Manager with assessment, plan of care changes, and ongoing needs.
  • Utilize teaching, assessment, and intervention skills to provide comfort care and maximize the quality of life for patients and families.
