Job Summary

We are seeking a dynamic and experienced Licensed Practical Nurse (LPN) to join our Home Health & Hospice team. As an LPN, you will play a crucial role in ensuring our patients receive high-quality care, working closely with physicians and registered nurses to develop and implement individualized care plans.

Responsibilities
  • Verify and execute care plans as prescribed by physicians and under the guidance of a registered nurse
  • Provide essential nursing treatments, address patient pain, and assist with activities of daily living
  • Administer medications, ensure supply availability, and maintain accurate patient records
  • Liaise with supervising RNs or physicians on patient assessments and condition changes
  • Contribute to team meetings to ensure cohesive patient care coordination
Requirements
  • Current Licensed Practical Nursing License in the state of practice
  • 1+ years of Home Health & Hospice experience preferred
  • Ability to work independently, be self-directed, and flexible
  • Ability to use a full range of motion and lift 35lbs
  • Effective verbal and written communication and time management skills
  • Valid Driver's License (clean record) and reliable transportation
  • Proof of current auto insurance and registration
  • Valid AZ Fingerprint Clearance Card (we can help)
  • Current CPR Certification (we can help) and Negative TB Test (we can test)
