Job Overview

This position involves providing comprehensive case management services to clients, connecting them with local resources and support systems, and collaborating with clinicians and other service providers to empower individuals to end their homelessness.

The successful candidate will have a strong understanding of human services, excellent communication skills, and the ability to work effectively with diverse populations.

Key Responsibilities:
  • Provide linkages for clients who need social, mental health, educational, vocational, recreational, spiritual, and or substance abuse services
  • Provide transportation to appointments for clients
  • Coordinate and collaborate with other agencies, funders, and social service providers
  • Oversee intake, departures and daily roster and programs
  • Attend weekly and monthly agency, program and network meetings

We offer a competitive salary of $52,400 - $57,600 per year, depending on experience, plus excellent benefits including medical, dental, and vision insurance, 401(k) retirement plan with employer matching contribution, paid time off, and 10 paid holidays.
