We are committed to creating a safe and supportive environment for our residents, and we are seeking a dedicated and compassionate Transitional Housing Program Coordinator to join our team.

This full-time position offers a competitive salary of $27.50 per hour, along with opportunities for professional growth and development in a dynamic and supportive environment.

The successful candidate will be responsible for delivering exceptional care and support to our residents, including intake and assessment, case planning, documentation, and workshops.

Key Responsibilities:

  • Provide case management services, including translation, speaking, and writing in Spanish
  • Develop and maintain individual service plans (ISPs) with youth
  • Document regular progress notes and complete documentation specific to external grants as required
  • Facilitate psycho-educational and support group activities with youth
  • Perform custodial supervision and ensure adherence to the daily program schedule

Requirements:

  • Bachelor's degree in Psychology, Sociology, Social Work or related field OR an Associate degree in Psychology, Sociology, Social Work or related field and 2 years of case management experience
  • Fluency in Spanish required
  • Self-starter and team player with excellent organizational skills and communication abilities
