Care Transition Coordinator Healthcare Specialist

The successful candidate will collaborate effectively with Case Managers to ensure smooth discharge planning processes for each patient encounter.
The estimated salary for this role is $62,400 - $74,000 per year, based on industry standards and location. If you have experience in acute care hospitals and possess the required skills and qualifications, we encourage you to apply.
About the Role:
We are looking for an experienced Care Transition Coordinator who can assess, plan, implement, coordinate, monitor, and evaluate options and services to provide a safe transition from acute care hospital. The ideal candidate will be responsible for:
- California Licensed Vocational Nurse License
- BLS Certification
- Clinical experience in acute care hospital preferred
