Job Title: Case Manager II

This is a challenging and rewarding role that requires a strong understanding of social services and established casework methods/techniques. The Case Manager II will work with highly vulnerable populations impacted by chronic physical, mental, substance use, and/or incarceration issues.

Key Responsibilities:
  • Interviews, counsels, and investigates clients' background to determine eligibility for community-offered services
  • Works with and coordinates services for individuals with a history of chronic homelessness who have exhibited significant behaviors that threaten their safety and well-being
  • Delivers services to individuals with one or several of the following: living with severe mental illness, developmental delays, emotional trauma, substance use disorder, and/or other disorders
  • Assists individuals in developing a self-sufficiency and/or stabilization plan, which is the foundation to maintaining housing
  • Conducts daily visits to assess living conditions/environments, engage, and maintain client to case manager relationship
  • Builds trust and rapport by maintaining ongoing contact with the individual, family, or other support members
  • Provides assistance in re-establishing or developing skills, such as life skills (hygiene, cleaning, budgeting, etc.)
  • Determines eligibility for all financial and support services that can assist client in meeting stabilization plan
  • Records all contacts, requests for assistance, dispositions, and referrals directly into the various case management systems
  • Participates in interdisciplinary teams that address both system and client specific situations
Requirements:
  • Bachelor's degree in human services or behavioral sciences, and Two years of experience in case management of chronically homeless individuals
  • Or any equivalent combination of related training and experience
  • Applicants within six months of meeting the minimum education/experience requirement may be considered for trainee status
  • A valid Florida Driver License is required and a Motor Vehicle Record that meets the requirements of Alachua County policy # 6-7
Working Conditions:

The work is performed under the direction of a higher-level supervisor and is reviewed through conferences, reports, and observation of the results obtained.
