Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at Mercy Health. As a key member of our interdisciplinary team, you will play a critical role in coordinating patient care and ensuring seamless transitions between treatments. About the Job- You will work collaboratively with healthcare providers to assess patients' needs and develop individualized care plans.
- Your focus will be on coordinating patient care, facilitating communication between healthcare teams, and ensuring timely and efficient delivery of services.
- Collaborate with patients and their families to provide education and support, promoting informed decision-making and empowerment.

Responsibilities- Assess patients' physical and emotional needs, identifying opportunities for coordination and collaboration.
- Develop and implement individualized care plans that address specific patient goals and objectives.
- Collaborate with healthcare providers to coordinate patient care and ensure seamless transitions between treatments.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
- Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction.
- You must hold a current RN license in the state where you will be working or be covered by compact.
- BLS Basic Life Support certification is required; ACLS Advanced Cardiac Life Support and Medical-Surgical Nursing Certification are preferred.
- You will need strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
A salary range of $90,000-$115,000 per year is offered for this position, based on experience and qualifications.
