End-of-Life Care Nurse

We are seeking a compassionate and skilled End-of-Life Care Nurse to join our team at Concho Hearts Hospice. As a member of our interdisciplinary group, you will play a vital role in providing symptom management, patient and family support, while coping with an end-of-life illness.
Job SummaryThe primary functions of this position include administering nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency; and assuming responsibility for coordination of care.
Responsibilities:- Developing Patient Care Plans: Prepare and adjust patient care plans as needed, ensuring that individualized psycho-social needs are met.
- Providing Support and Education: Provide loved ones and primary caregivers with patient care information, end-of-life education, and emotional support.
- Managing Medical Records: Maintain and update medical charts, patient files, and health records accurately and efficiently.
- On-Call Rotation: Participate in an on-call rotation for weekdays and weekends, ensuring seamless coverage and prompt response to patient needs.
- Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
- Licensure: Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC), current Drivers License.
- Experience: Two years experience as a Registered Nurse in a clinical care setting preferred, hospice preferred.
- Skills: Nursing skills as defined as generally accepted standards of practice, good interpersonal skills, and ability to work under stressful conditions to meet deadlines and patient needs.
