About the Role:

We are seeking a skilled Registered Nurse to join our team at Concho Hearts Hospice. In this rewarding position, you will have the opportunity to provide compassionate care to patients and their families during some of life's most challenging times.

The primary function of this role is to administer nursing care for patients of all ages in their place of residence, coordinating care with the interdisciplinary group, patient, family, and referring agency.

The ideal candidate will have excellent interpersonal skills, with the ability to work independently and as part of a team. They will also be able to maintain accurate medical records and communicate effectively with patients and their loved ones.

Responsibilities:
  • Develop and implement individualized patient care plans that meet the unique needs of each patient.
  • Provide emotional support and education to patients and their families, empowering them to make informed decisions about their care.
  • Maintain accurate and up-to-date medical records, including patient charts and health records.
  • Participate in on-call rotations to ensure 24/7 coverage for patients.

Requirements:
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Current license as a registered nurse (RN) and/or by the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • Two years of experience as a Registered Nurse in a clinical care setting, preferably in hospice care.
  • Excellent nursing skills and the ability to work in a fast-paced environment.
