Job Summary

Fresenius Medical Care North America is seeking a highly skilled and compassionate Registered Nurse to join our team as a Home Therapies Registered Nurse for our Transitional Care Unit (TCU). As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Provide comprehensive nursing care to patients undergoing dialysis in their homes, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Conduct regular home visits to assess patient progress, provide education, and address any concerns or issues that may arise.
  • Work closely with patients and their families to ensure a smooth transition from hospital to home care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in home dialysis care.
Requirements
  • Current RN licensure in the state of employment.
  • Minimum of 2 years of experience in nephrology nursing, with a focus on home dialysis care.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
  • Basic computer skills and proficiency in electronic medical records.
