Job Summary

As a key member of our Transitional Care Unit, you will play a vital role in providing exceptional patient care and support to individuals with end-stage renal disease.

Key Responsibilities
  • Provide comprehensive patient care, including assessment, planning, intervention, and evaluation, to ensure optimal health outcomes.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Administer medications and treatments as prescribed, and document patient responses and progress.
  • Conduct regular patient education and training on dialysis therapies and self-care techniques.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Supervise and mentor new staff members, providing guidance and support to ensure successful onboarding.
  • Stay up-to-date with industry developments and best practices, applying knowledge to improve patient care and outcomes.
Requirements
  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Current or successful completion of CPR BLS Certification.
  • Minimum of 2 years of Nephrology Nursing experience within the last 2 years as a RN.
Preferred Qualifications
  • Experience with home dialysis therapies and patient education.
  • Knowledge of quality improvement principles and practices.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
