Position: Advanced Practice Provider - Indiana Region
*Sign-On Incentive: $20,000
**Candidate must hold a valid license in the state of Indiana.
Location: This position requires travel to provide care in various communities within Indiana.
*Eligible for mileage reimbursement.
Work Schedule: Full-time, Monday to Friday, 40 hours per week.
The Advanced Practice Provider plays a vital role in collaborating with healthcare professionals, including physicians and other providers, to formulate comprehensive care plans tailored to the patient's health status and personal objectives. This role encompasses both clinical and supportive functions for patients receiving palliative care within a designated area.
Key Responsibilities:
- Deliver care services in home environments to meet patient needs.
- Design and execute clinical care plans for adults dealing with chronic and intricate health issues.
- Conduct thorough assessments and diagnostics to create and implement treatment strategies aligned with patient goals.
- Address the physical and emotional needs of patients and their families, supported by an interdisciplinary team.
- Provide education to patients and families regarding medication management, health conditions, nutrition, and preventive care.
- Coordinate with other healthcare team members and external providers to ensure comprehensive care and resource availability.
- Maintain accurate medical records and documentation in compliance with state regulations and company policies.
- Engage in ongoing professional development as mandated by state requirements.
- Prescribe medications in accordance with state regulations.
Minimum Qualifications:
- Must possess a current, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in Indiana.
- DEA registration or eligibility is required.
- CPR certification is mandatory.
- Completion of a Tuberculosis test is a prerequisite for this role.
Preferred Qualifications:
- At least 2 years of experience in home healthcare, complex case management, or palliative care is preferred. Experience in hospital settings, oncology, emergency care, or intensive care is highly valued.
- Familiarity with Electronic Medical Records (EMR) systems is advantageous.
