As a Licensed Practical Nurse (LPN), you will work under the supervision of a Registered Nurse to deliver skilled nursing care, develop care plans, and coordinate with healthcare teams to ensure the best possible outcomes for our patients.
Responsibilities:- Deliver skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure comprehensive care.
- Provide education and support to patients and their families to promote health and wellness.
- Document patient care and progress in a timely and accurate manner.
- Current, unrestricted LPN license in the state of practice.
- At least 6 months of experience as an LPN in a clinical care setting.
- Current CPR certification.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
