Job Title: LPN Pediatric Home Health Nurse

We are seeking a compassionate and skilled LPN to join our team of dedicated healthcare professionals at Angels of Care Pediatric Home Health. As a LPN, you will provide high-quality, patient-centered care to children and young adults in their homes, making a meaningful difference in their lives.

About the Role:
  • Provide direct, one-on-one medical care to patients in their homes, focusing on their medical needs and treatment.
  • Conduct ongoing patient assessments and administer prescribed medication, treatments, and therapies.
  • Coordinate care with other healthcare professionals to ensure seamless patient outcomes.
  • Educate family members on patient clinical care to enhance positive outcomes.
  • Preventative initiatives to protect quality of care for patients.

Requirements:
