Patient Care Coordinator RN

The Wilson Medical Center is seeking a highly skilled Registered Nurse to fill the position of Patient Care Coordinator RN. As a member of our healthcare team, you will be responsible for coordinating patient care activities, ensuring seamless delivery of services, and promoting patient satisfaction and safety.
Duties and Responsibilities:
- Coordinate patient care activities, including assessing patient needs and developing individualized care plans.
- Collaborate with the healthcare team to ensure effective communication and coordination of care.
- Participate in patient and family education, providing information and support to empower patients to make informed decisions about their care.
- Foster strong relationships with colleagues, maintaining open communication and teamwork to achieve exceptional patient outcomes.
Requirements:
- A minimum of 2 years' experience as a Registered Nurse in an acute care setting.
- Current active North Carolina or compact RN license.
- BLS certification required; ACLS certification preferred.
