Job Summary

The University of Findlay is seeking a highly skilled and motivated Population Health Fellow to join our Medication Therapy Management Telehealth Center. As a key member of our team, you will play a critical role in providing population health services and working closely with our population health specialists to develop and implement population health strategies.

Key Responsibilities
  • Operate primarily from the University of Findlay Telehealth Center, providing population health services with occasional travel to Mercy Health sites within Ohio and Kentucky.
  • Work closely with population health specialists to develop and implement population health strategies, with a focus on medication therapy management, chronic care management, and comprehensive primary care plus activities.
  • Conduct medication therapy management sessions, including comprehensive medication reviews, personal medication records, medication-related action plans, interventions, and referrals.
  • Supervise graduate assistants, student employees, and pharmacists, providing guidance and support to ensure the successful completion of projects and tasks.
  • Develop and refine policies and procedures related to the operation of the Medication Therapy Management Telehealth Center.
  • Ensure compliance with all relevant laws, regulations, and accreditation standards, including CMS billing, fraud waste and abuse, and HIPAA requirements.
  • Perform other duties as assigned by management, including interviewing and hiring student employees and non-faculty pharmacists, conducting performance appraisals, and renewing the State Board of Ohio Terminal Distributor/Consulting Pharmacy license annually.
Requirements
  • Doctor of Pharmacy degree from an accredited institution.
  • Current licensure as a registered pharmacist in the State of Ohio with no restrictions on license.
  • Personal professional liability insurance.
  • Eligibility to serve as a preceptor for pharmacy students in the State of Ohio.
  • Ability to work in and understand the academic environment.
  • Skill in program development and execution.
  • Demonstrated commitment to the principles of diversity.
  • Excellent communication and presentation skills.
  • Excellent interpersonal and communication skills, and ability to work well with other pharmacists, health care providers, and students.
  • Possession of a personal NPI number.
  • Ability to sign and abide by the Employer's Non-Disclosure Agreement.
  • No pharmacy license restrictions imposed by the Ohio State Board of Pharmacy.
Preferred Qualifications
  • APhA Medication Therapy Management Certificate training.
  • Experience with OutcomesMTM/Mirixia MTM platform.
