Hospice On-Call Nurse Opportunity

At Advanced Home Health, we seek a skilled and compassionate Hospice On-Call Nurse to deliver high-quality patient care as needed while on-call. Reporting directly to the Branch Clinical Manager, this role requires collaboration with local and regional leadership, as well as Interdisciplinary Teams.

Key Responsibilities:
  • We expect our ideal candidate to spend quality time with patients and their families, supporting everyday needs, conducting comprehensive OASIS assessments, developing personalized care plans, and providing skilled nursing care according to these plans.
  • The successful candidate will also perform assigned duties, including medication administration, wound care, treatments, and procedures, all while practicing knowledge of Medicare CoP's, proficient documentation skills, attention to detail, prioritization, and effective time management.
  • Evaluating the effectiveness of services provided to each patient, consulting with physicians as necessary, maintaining consistent and timely charting in the Electronic Medical Record, monitoring clients' conditions, and reporting changes to the Clinical Manager and Physicians are also essential responsibilities.
  • Additionally, our Hospice On-Call Nurse will provide case management and care coordination while in the field, supported by the clinical manager, ensuring that care adheres to Federal and State guidelines, meeting full-time minimum visit expectations per established metrics, and being available on-call at all times during work hours.
Requirements:
  • A Registered nurse with current licensure to practice professional nursing in the state of practice is required.
  • Demonstrated ability to supervise and direct professional administrative personnel is also essential.
  • Experience in case management, disease management, home health care nursing, hospital nursing, or intensive outpatient education is highly valued.
  • A licensed driver with an automobile that is insured in accordance with state and/or organization requirements is also necessary.
