About the Role

As a Recovery Support Specialist at Community Health Resources, you will play a vital role in helping individuals with behavioral health needs achieve their recovery goals. With a strong focus on community resources and culturally informed practices, you will assist clients in identifying and advocating for their own recovery strategies.

Key Responsibilities
  • Work collaboratively with hospital-based staff, EMS, law enforcement, and clients' natural supports to provide comprehensive care.
  • Use your lived recovery experience to assist clients in identifying and advocating for their own recovery goals.
  • Provide independent and/or co-response with mobile clinical staff on crisis outreaches and follow-up visits to promote client-centered, recovery-based approaches.
  • Participate in interventions to help reduce healthcare disparities, including screening for Social Determinants of Health (SDOH) such as housing, food security, and transportation.
  • Link clients to behavioral health follow-up care, natural supports, and community resources, including LGBTQ+, Veteran's, and other support groups, in a culturally informed, strength-based manner.
  • Provide follow-up engagement support, including brief recovery coaching, relapse prevention, and safety planning, as well as inclusion of family involvement where appropriate.
  • Collaborate with individuals to identify treatment options and assist in positive engagement with new or existing treatment providers.
  • Utilize best practices, including motivational interventions, to assist clients in meeting their goals and moving along their paths to recovery.
Qualifications

We are seeking an individual with a strong background in recovery support or coaching, preferably with a Bachelor's degree or equivalent education and/or experience. A valid driver's license and use of a personal vehicle may be required for the position.
