We are seeking an experienced Physical Therapist to join our Home Health team in Roanoke, VA. As a Physical Therapist, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to develop and implement individualized treatment plans.
Responsibilities:- Provide physical therapy services to patients in their homes, including assessments, evaluations, and treatment plans.
- Implement and conduct treatment as outlined in the treatment plan, using evidence-based practices and techniques.
- Treat patients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments and procedures.
- Monitor patient's response to treatment and modify treatment as indicated in accordance with physician's orders.
- Communicate evaluation findings to patients, family, and other staff members.
- Valid Physical Therapy license in the state of VA.
- Valid Driver's License.
- Current CPR certification.
- Minimum of one year verifiable physical therapy experience.
- Home Health experience preferred.
