VNA Health Group is seeking a skilled and compassionate Registered Nurse to join our home health team in Rutherford, New Jersey.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to provide high-quality patient care in the comfort of their own homes. As a Home Health Registered Nurse, you will be responsible for assessing patient needs, developing individualized care plans, and delivering evidence-based care to patients with multiple disease processes and co-morbidities.

Responsibilities:
  • Assess and evaluate patient needs, developing and implementing individualized care plans
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with healthcare providers and other members of the interdisciplinary team to ensure comprehensive care
  • Document patient progress and outcomes, updating care plans as needed
  • Identify teaching needs and develop strategies for patient and family education
Requirements:
  • RN with Bachelor's degree in Nursing preferred
  • 1+ year of supervised clinical experience as a Registered Nurse
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
