We are proud to offer a Nurse Admissions Specialist position at Sangre de Cristo Hospice & Palliative Care, where you will make a meaningful difference in the lives of patients and their families.

As a key member of our interdisciplinary team, you will assess and admit patients to our hospice program, ensuring they receive comprehensive care and support. Your expertise will enable patients to live their final days with dignity and comfort.

About the Role:

  • Assess and admit patients to our hospice program
  • Develop and implement patient care plans in collaboration with the Interdisciplinary Team
  • Optimize patient comfort and quality of life through comprehensive care and support

We offer a competitive salary range of $75,000 - $90,000 per year, depending on experience. Our benefits package includes:

Requirements:

  • Valid non-restricted RN license in Colorado
  • Valid non-restricted driver's license
  • Flexibility to work a weekend schedule if needed
  • At least 1 year of hospice or home health experience preferred
