Job Description

We are seeking an experienced Intake RN Case Manager to support our dynamic and growing team in Chicago. As an essential part of our mission-driven organization, you will work closely with underserved populations, helping to prevent complications and reduce health disparities within our community.

This role is ideal for someone who is passionate about making a difference and who thrives in a flexible, collaborative environment.

Key Responsibilities:
  • Patient Interaction & Case Management: Engage with patients directly, both in the field and virtually, to manage their care effectively.
  • Field Work & Office Duties: Spend a significant amount of time in the field, visiting patients' homes. When not in the field, you will be based in our Chicago office, conducting virtual visits and collaborating with the care management team.
  • Collaborative Partnership: Build and maintain strong relationships with key partners, including hospital case management teams and primary care providers, to support patient engagement and improve health outcomes.
  • Census Management & Coordination: Review daily censuses, manage inpatient, ER, and skilled nursing facility stays, and coordinate transitions of care with providing partners.
  • Outreach & Support: Perform outreach to hospital case managers and patients, providing telephonic support as needed. Assist with new patient engagement and ensure smooth transitions and continuity of care.
Qualifications:
  • Experience: At least 5 years of relevant clinical experience, with a minimum of 3 years in care management within a health plan, home health, or hospice setting.
  • Background: Strong case management experience on the provider or payor side, with a focus on patient interaction rather than utilization review.
  • Education: Bachelor of Science in Nursing (BSN) required. Active IL RN license is mandatory.
  • Flexibility: Ability to adapt to a fast-paced, evolving environment. This is a growing organization where processes may change, and flexibility is key.
  • Mission-Driven: A genuine desire to give back to underserved populations and a commitment to helping prevent health-related complications and deaths.
  • Self-Motivated: Ability to work independently without much oversight, while still being a collaborative team player.
