Job Summary:The Discharge Coordinator will provide critical support to our psychiatric case-management department, ensuring a smooth transition for patients from our facility to their next level of care. This role requires strong communication and organizational skills, as well as the ability to work effectively with patients, families, and healthcare providers.
Key Responsibilities:- Initiate discharge planning and arrange aftercare appointments/referrals.
- Complete all necessary discharge paperwork with patients and support staff at time of discharge.
- Conduct psychosocial assessments on new patients within required timelines.
- Engage in collateral contacts with community organizations, support persons, and other providers, documenting contacts as needed.
- Attend team meetings as required.
- Bachelor's degree in Social Work, Psychology, or a related field.
- Previous behavioral health experience preferred.
- Case management and care coordination experience preferred.
