Job Summary

This is a challenging and rewarding opportunity to work as a Case Manager I with Community Health Plan of Washington (CHPW). As a key member of our team, you will be responsible for providing case management services to our members with complex medical and behavioral health needs.

Key Responsibilities
  • Assess and evaluate members' health status, identify care needs, and ensure access to appropriate services to achieve positive health outcomes.
  • Develop and implement individualized care plans in collaboration with members, families, and healthcare teams.
  • Coordinate with healthcare providers, community resources, and health plan partners to ensure seamless care coordination.
  • Monitor and evaluate members' progress, identify potential barriers to care, and make adjustments to care plans as needed.
  • Document all case activity using our care management system and follow documentation standards and protocols.
Requirements
  • Bachelor's degree in nursing or a related field (master's degree in social work preferred).
  • Current, unrestricted license in the State of Washington as a Mental Health Counselor (LMHC), Mental Health Professional (LMHP), or Marriage and Family Therapist (LMFT).
  • Minimum one year of case management, home health, or discharge planning experience; or equivalent combination of education and experience.
  • Experience working with individuals with disabilities and knowledge of Child and Family Services.
  • Minimum three years of clinical experience in an acute care and/or outpatient setting.
  • Experience and proficiency with Microsoft Office products.
  • Bilingual abilities (preferred).
