About the Role

We are seeking a highly skilled and compassionate Case Manager I to join our team at Community Health Plan of Washington (CHPW). As a Case Manager I, you will play a critical role in delivering high-quality care management services to our members, ensuring they receive the necessary support and resources to achieve optimal health outcomes.

Key Responsibilities
  • Provide telephonic case management services to members with acute, chronic, and complex needs.
  • Assess members' health status, identify care needs, and ensure access to appropriate services to achieve positive health outcomes.
  • Collaborate with healthcare teams, providers, and community resources to develop and implement care plans that meet members' unique needs.
  • Conduct regular assessments, evaluations, and revisions of care plans to ensure members receive the most effective and efficient care.
  • Communicate effectively with members, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Document all case activity using CHPW's care management system and adhere to documentation standards and protocols.
Requirements
  • Bachelor's degree in nursing, social work, or related field (or equivalent experience).
  • Current, unrestricted license in the State of Washington as a registered nurse (RN), social worker (LSWAA, LSWAIC), or mental health professional (LMHC, LMHP, LMFT).
  • Minimum one year of case management, home health, or discharge planning experience (or equivalent combination of education and experience).
  • Minimum three years of clinical experience in an acute care and/or outpatient setting.
  • Experience with Microsoft Office products and care management software applications.
  • Case Management Certification (preferred).
  • Bilingual abilities (preferred).
