At Resiliency LLC, we offer a competitive salary of $90,000 - $110,000 per year for our Care Manager LCSW role in Torrance and Santa Monica, CA.

We are seeking an experienced Care Manager to join our team at our Health Center. As a Care Manager, you will play a vital role in assessing and coordinating care for patients in the community. This is a full-time variable shift position with day and evening hours.

The ideal candidate will have a Master's Degree in Social Work from an accredited program and be licensed as a California Clinical Social Worker. You will also have experience working in healthcare, preferably in acute or post-acute settings, and have knowledge of case management principles and practices.

Our hospital values collaboration and communication among our multidisciplinary team members, including nurses, social workers, physicians, and other support staff. We strive to provide high-quality, patient-centered care that addresses the complexities of care, including social determinants of health and psychosocial issues.

To succeed in this role, you must possess excellent assessment and coordination skills, as well as strong communication and interpersonal skills. You should be able to work independently and as part of a team, think critically, and prioritize tasks effectively.
