Job Summary

We are seeking a highly skilled Resource Specialist to join our team at SPECTRAFORCE. As a Resource Specialist, you will play a critical role in providing care management services to assist with coordination of care, including assessing for healthcare needs, referring to health plan providers, providing education on benefits, and connecting individuals to community resources to address social determinants of health needs.

Key Responsibilities
  • Provide field-based care management services to a caseload of 100 members, including assessing for healthcare needs, referring to health plan providers, and connecting individuals to community resources.
  • Document all interactions and care plans in an electronic health record.
  • Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless care coordination.
  • Develop and implement care plans that address the unique needs of each member.
  • Provide education and support to members on benefits, healthcare options, and community resources.
Requirements
  • Bachelor's degree in a related field.
  • Minimum 2 years of experience in pediatric case management, health plan experience, or a related field.
  • Strong knowledge of community resources and social determinants of health.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Preferred Qualifications
  • Pediatric case management experience.
  • Health plan experience.
  • Experience connecting families and children to local resources for various services, including employment, housing, independent living, justice, foster care, and adoption.
