Job Summary

Optum is seeking a skilled Home Infusion Nurse to join our team. As a Home Infusion Nurse, you will provide clinical care and education to patients receiving parenteral medications in the comfort of their own homes.

Key Responsibilities
  • Establish and Maintain Positive Patient Relationships: Develop and maintain strong relationships with patients and their families, providing education and support to ensure successful infusion therapy outcomes.
  • Assess Patient Appropriateness: Evaluate patients' medical history, safety of their home environment, and availability of caregiver support to determine suitability for home infusion therapy.
  • Complete Clinical Documentation: Accurately and thoroughly document patient information, including medical history, treatment plans, and medication profiles.
  • Participate in Case Conferences: Collaborate with the multidisciplinary care team to develop comprehensive care plans and ensure seamless patient care.
  • Provide Skilled Care: Administer medications, establish and maintain vascular access devices, and perform therapy-specific clinical assessments in accordance with company policies and protocols.
  • Communicate Effectively: Share observations and patient responses with the care team, ensuring timely and effective communication to ensure optimal patient outcomes.
Requirements
  • Active RN Licensure: Possess an active and unrestricted Registered Nurse (RN) license in the state(s) of practice.
  • Basic Life Support Certification: Hold current Basic Life Support (BLS) and/or CPR certification, or be willing to obtain prior to starting employment.
  • Infusion Therapy Experience: Possess at least 1 year of experience working as an RN in a clinical care setting that contributes to proficiency in infusion therapy.
  • Electronic Medical Record Proficiency: Be proficient with electronic medical record documentation.
  • Interdisciplinary Teamwork: Demonstrate ability to work collaboratively with an interdisciplinary team of professionals, including pharmacists, technicians, physicians, and registered dietitians.
Preferred Qualifications
  • Home Infusion Experience: Possess experience administering infusion therapy in a home or alternate-site setting.
  • Microsoft Office Proficiency: Be proficient with Microsoft Office and electronic medical records (EMR) programs.
  • Infusion Nursing Society Knowledge: Possess knowledge of the Infusion Nursing Society (INS) standards of practice, policies, and procedures.
